GoodLife Fitness New Membership Reward Program

Rules and Regulations
Sign up for a new, one year GoodLife Fitness membership for yourself and/or for a member of your immediate family between September 1, 2017 and February 28, 2018 and be eligible for one (1) of two (2) reward packages, each package consisting of one (1) iPad 32GB WiFi and one (1) free one year GoodLife membership, compliments of Edvantage (the “New Membership Reward Program”) or (“Reward Program”). Please read the rules and regulations below.
Each reward package consists of one (1) iPad 32GB WiFi and one (1) free, one year basic GoodLife membership (“reward package”) valued at $848 + applicable taxes (iPad = $449.00 and one-year GoodLife membership = $399.00, excluding taxes. Taxes are applied by the Edvantage member’s residential province). The free membership to GoodLife can be:
- Given to another Edvantage member to use at a GoodLife location of choice, or
- Given to a spouse or dependent (“dependent” defined as being a son, daughter, step-son, step-daughter and/or a dependent person all who are between the ages of 12 and 25 and live at the same residential address as the Edvantage Member) to use at a GoodLife location of choice, or
- Alternatively, added to the end of the selected Edvantage Member’s current membership at a GoodLife location of choice.
If the selected Edvantage Member gives away the GoodLife membership the recipient must add the membership to the end of their current GoodLife membership thereby extending their current membership for one (1) year. If the recipient does not have a current GoodLife membership, the recipient can use the membership as a new membership. If the Edvantage Member gives away the one year GoodLife membership, the membership will be administered at the GoodLife location of choice. If the Edvantage Member keeps the one year GoodLife membership for themself, the membership will be administered by Edvantage. Any upgrades, administration fees, extensions to the GoodLife membership rewarded are at the cost of the person using the membership. The iPad 32GB WiFi comes “as is”. Any upgrades, connection charges, service and provider costs are at the cost of the person receiving or using the iPad. Only one reward package per Edvantage Member, per family, per household or per address will be awarded.
This Reward Program is open to all education employees who are valid and current Edvantage Members, 18 years of age or older, resident of Ontario or resident of Alberta with a valid email address who purchase a new, one year GoodLife membership in Ontario or Alberta for themselves and/or for their spouse and/or dependent(s). The New Membership Reward Program is only for first-time, new GoodLife memberships and excludes anyone renewing and/or upgrading and/or downgrading an existing GoodLife membership.
Two (2) selected Edvantage Members will each receive a reward package for signing up and paying for a new, one year membership to a GoodLife Fitness Club in Ontario or Alberta for themselves and/or their spouse and/or their dependent(s) between September 1, 2017 and February 28, 2018. Edvantage Members who purchase a new GoodLife membership will receive one (1) chance for the reward package for every new membership registered under the Member’s Edvantage number provided all new members belong to the Member’s immediate family (spouse and/or dependent(s)). All memberships purchased must be new memberships, paid in full and cannot be a renewal of an existing membership, an upgrade or a downgrade of an existing membership for the Member to be eligible for a chance under the Reward Program. The membership has to be purchased through the primary Member’s Edvantage number through a site protected by the Member’s login information. Spouses and/or dependents of Edvantage Members are not eligible to enter the Reward Program on behalf of an Edvantage Member unless they are valid Edvantage Members.
Chances of receiving the reward package do not increase by adding upgrades to the new GoodLife membership or by signing up for more than one (1) year for any new membership.
The New Membership Reward Program excludes employees, directors and trustees of OTIP, TW Insurance Brokers Inc. (“TW Insurance”) and their affiliated companies1, the reward selection team and members of the respective immediate families of OTIP and TW Insurance, employees, directors and trustees and/or anyone with whom the OTIP and TW Insurance employees, directors and trustees are domiciled. Employees of GoodLife and any of its affiliates, parent companies, subsidiaries, advertising and promotional agencies, and their immediate family members or household members of each are also excluded.
How to Enter
For online new memberships, this Reward Program begins at 12:01:00 a.m. ET on Friday, September 1, 2017 and closes at 11:59:59 p.m. ET on Wednesday, February 28, 2018 ("membership closing date"). For telephone memberships, this New Membership Reward Program begins at 9:00:00 a.m. ET on Friday, September 1, 2017 and closes at 5:00:00 p.m. ET on Wednesday, February 28, 2018 ("membership closing date by phone").
To purchase a new GoodLife membership online, visit www.edvantage.ca/share-the-goodlife to complete and submit the online form. To purchase by telephone, call Edvantage Customer Service at 1-866-750-6847. Members who purchase their membership through any other website, including GoodLife’s website, or by calling any other number other than the Edvantage Customer Service phone number, or who purchase their memberships at a GoodLife facility will not be eligible for the New Membership Reward Program.
Members will be required to provide their personal information to purchase their GoodLife membership, including but not limited to, their name, Edvantage number, the name of the person registering for the new membership, address, contact information (“mandatory information”). By purchasing online through www.edvantage.ca/share-the-goodlife, Edvantage Members agree that Edvantage may use the Member’s name, telephone number, email address and/or residential address entered into the GoodLife New Membership screens to administer this New Membership Reward Program should the Member’s name be selected as the recipient of the reward package. A valid Edvantage number and valid MasterCard or Visa account number with expiry date and security code (3 digit number on the back) will be required to purchase the membership. American Express cards will not be accepted. Payment for each membership purchased must be successfully processed and cleared for payment by the credit card company before the Member becomes eligible for the New Membership Reward Program. Payment by cheque or by cash is not accepted. Members who purchase their membership(s) online by the membership closing date or membership closing date by phone will receive one chance for every new membership purchased in conjunction with the terms and conditions of the Reward Program and all chances will automatically be entered into the New Membership Reward Program to be eligible for one (1) of two (2) reward packages. All new GoodLife memberships under a Member’s Edvantage number must be registered to the same residential address as the Member.
Entries are limited to one entry per person per GoodLife one year new membership. Duplicate entries will be declared void. Members who purchase their membership(s) online by the membership closing date or membership closing date by phone will receive one chance to receive one (1) of the two (2) reward packages. GoodLife memberships are non-refundable and cannot be transferred, placed on hold, or cancelled prior to the membership expiry date.”
There will be two (2) separate and random draws for the reward packages during the Reward Program. The first random draw for one (1) reward package will take place on or about November 30, 2017 from all eligible GoodLife new memberships purchased online and by phone, combined, by November 30, 2017. The second draw for one (1) reward package will take place on or about March 14, 2018 from all eligible GoodLife new memberships purchased online and by phone, combined, between December 1, 2017 and February 28, 2018. Each reward package will be given to the selected Member who belongs to the Edvantage number registered with the membership.
The random draws will take place in Waterloo, Ontario. Odds of receiving the reward package are dependent on the number of eligible new memberships purchased online and by phone by the first draw date of November 30, 3017 and then received between December 1, 2017 and the contest closing date. All decisions by the selection team are final on all matters relating to the New Membership Reward Program. Neither OTIP, TW Insurance, Edvantage or GoodLife shall be held responsible for any errors or negligence that may arise or occur in connection with the New Membership Reward Program including, but not limited to, the Edvantage Member’s information (whether it be mandatory information or optional information) entered into the GoodLife membership registration form by any person, including a customer service representative. For information on the winners of the reward packages, visit the Edvantage website at www.edvantage.ca.
The selected Edvantage Members will be notified by telephone, by email or by regular mail using the information from the GoodLife membership form. If a reward notification is returned to Edvantage as undeliverable, the reward will be awarded to an alternate Edvantage Member who entered the Reward Program. If the selected Edvantage Member cannot be reached within two (2) weeks from the first attempt at notification, declines their reward or fails to return a release of liability with ten (10) days’ time to Edvantage, their opportunity to receive the reward will be declared ineligible, forfeited and another Edvantage Member’s name will be selected.
GoodLife Memberships are subject to verification and will be declared invalid if they are illegible, incomplete, mechanically reproduced, mutilated, forged, falsified, altered or tampered with in any way.
The reward package, including the iPad will be couriered to the selected Edvantage Member. Post office box addresses are not acceptable as courier companies cannot deliver to box numbers. If the selected Edvantage Member resides at a country address and/or a rural route address, they may be asked for their emergency-locate address or other identifiable information to assist the courier company with the delivery of the reward package. The reward package will be delivered approximately two (2) weeks after each Edvantage Member is confirmed and declared the recipient, save and except for any unforeseen events such as, but not limited to, a weather event, postal or courier strike, flood, fire, war, break out of disease, quarantine, hydro outage, service provider problems. No cash value or other credit will be issued in lieu of the reward package.
Each reward package is non-transferable and non-exchangeable for cash or other rewards. The reward will be given to the Edvantage Member, even if the spouse’s and/or dependent’s name was selected. Any and all taxes on the reward package, not specifically mentioned in these rules, including income taxes, and any incidental expenses associated with collection of the reward are the responsibility of the Member. No substitutions will be made for the reward except at the discretion of Edvantage and/or GoodLife.
Edvantage reserves the right to withdraw, terminate, suspend, cancel, amend or modify all or part of the New Membership Reward Program and the contents of the reward package, including the number of reward packages in any manner and any rule(s) of the New Membership Reward Program, at any time and without prior notice, for any reason whatsoever or if it determines that for any reason the Program cannot be run as originally planned, including, but not limited to, technical failure or corruption, computer virus, bugs, computer, electronic or system malfunction, tampering, unauthorized intervention, fraud or any other factor(s) or cause(s) beyond Edvantage’s control which in Edvantage’s opinion compromises the fairness or integrity of the Reward Program and/or affects the administration, security, or proper conduct of this Reward Program as contemplated by these official rules.
If, as a result of an error relating to the entry process, entry, entry count, drawing or any other aspect of the New Membership Reward Program, there are more potential winners of the reward packages than contemplated in these rules, there will be a random draw amongst all eligible Members after the membership closing date to award the reward package.
By submitting a completed New Membership form for a GoodLife membership, and providing information to enter this New Membership Reward Program, the recipient of the reward package releases OTIP, TW Insurance, Edvantage and GoodLife from any and all liability arising out of, or in connection with, this Reward Program and the awarding of any reward package, now or in the future. By accepting the reward package under the New Membership Reward Program, the Edvantage Member consents to the use of their name in a format approved by that winner, city and province where they reside, audio/video image, statements, and/or photograph in any related publicity and publication without further compensation, worldwide and in perpetuity, in any and all forms of media and social media now known or hereafter devised, including, but not limited to, the Internet, Facebook, Twitter, Linked In, You Tube, Pinterest, SnapChat in any publicity or advertising carried out by Edvantage, unless prohibited by law. Edvantage reserves the right to post or publish the selected Edvantage Member’s name, city, or province of residence, photograph, likeness, voice or other statements on its website and/or on the website of Edvantage’s affiliated companies and/or on the sponsor’s website for advertising and promotional purposes, where lawful, without further compensation to the Edvantage Member.
The Edvantage Member receiving the reward package agrees to sign a form of declaration confirming compliance with the New Membership Reward Program rules and regulations and agrees to sign an indemnity release and waiver form forever discharging Edvantage, OTIP, TW Insurance and their affiliated companies, directors, trustees, officers and employees of OTIP and TW Insurance, the selection team and any of their parents, affiliates, agents, and contractors, including advertising and promotional agencies whether for the Reward Program and/or any of the reward packages, and their directors, trustees, officers and employees, from any and all action, proceedings, suits, liability, losses, costs, expenses (including legal fees and disbursements), damages, demands and claims of any kind, in respect of anything done by Edvantage and/or OTIP and/or TW Insurance in the performance of their duties hereunder in connection with the Reward Program and/or resulting from the acceptance, possession or use of any reward package, including without limitation, but not limiting to, any loss, personal injury, death, home damage, or claims based on publicity rights, defamation or invasion of privacy that could result from, but not limited to, the awarding of the reward package or use of the membership by signing an Agreement / Release Form as a condition to and in advance of receiving their reward package in favour of Edvantage/OTIP/TW Insurance such that Edvantage/OTIP/TW Insurance shall not be liable for the use or misuse of the GoodLife membership. Edvantage /OTIP/TW Insurance assumes no responsibility for the participation in the New Membership Reward Program or any liability, claims, demands, actions, loss, damage or personal injury that may be sustained during the Reward Program or during use of the reward package. The provisions of this section shall survive the termination of this New Membership Reward Program and may be relied upon by the parties hereto at any time.
Failure to comply with the terms of the New Membership Reward Program may result in disqualification from the Reward Program.
Edvantage, OTIP, TW Insurance and GoodLife are not responsible or liable for lost, late, damaged, destroyed, corrupted, garbled, delayed, illegible, incomplete, stolen, misaddressed or misdirected registrations of memberships nor for any problems or technical malfunctions of any telephone network or lines, cables, computer online systems or servers, computer software problems or traffic congestion on the Internet or at any website or on any phone lines or at any cell towers, network, satellite, electronic or hardware or software malfunctions, failures, connections, traffic congestion, labour unrest, or unauthorized human intervention, or the incorrect or inaccurate capture of entry or other information, or the failure any such information and assumes no liability for damage to any entrant’s or any person’s computer resulting from participating in the New Membership Reward Program. Further, and for greater certainty, GoodLife memberships that are improperly addressed, illegible or which do not precisely conform to the prescribed requirements will be considered invalid and void.
Edvantage, OTIP, TW Insurance and/or GoodLife are not responsible for any theft, destruction or unauthorized access to, or alteration of, entries; failures or malfunctions of electronic data transmission, telephones, telephone lines or telephone systems, interrupted or unavailable network, server or other connections, any error, omission, interruption, defect or delay in any transmission or communication; traffic congestion on the Internet or for any technical problem, including but not limited to any injury or damage to the participant’s or any other person’s computer related to or resulting from participation in this New Membership Reward Program; the cancellation, suspension or preemption of games, injuries or timing errors, electronic or equipment failure or other communication problems occurring in connection with participation in this Reward Program nor is it responsible for any transmission of viruses from the Edvantage web site to a computer system or if any whole entry system crashes resulting in the Reward Program being cancelled or other problems or errors of any kind whether mechanical, human, electronic or otherwise.
A reward package shall be void if the selected Edvantage Member does not take their reward package by December 31, 2018.
In the event of a dispute as to the identity of a winner based on an email address, the winning entry will be deemed to be made by the Edvantage Member on the GoodLife membership or associated with the GoodLife membership at time of entry.
Edvantage, OTIP, TW Insurance and GoodLife and the reward selection team and any of their parents, affiliates, agents, and contractors, including advertising, sponsoring and promotional agencies, and their directors, trustees, officers and employees expressly disclaim and shall be held free of all responsibility or liability for any act, error, omission, injury, loss, accident, delay or the neglect or default of any company or person providing any service for the New Membership Reward Program, whether through providers negligence or otherwise, or due to epidemic, social or political disturbances or due to any cause whatsoever occurring during the Reward Program and the use of the reward package.
It is the sole responsibility of any Edvantage Member to notify Edvantage’s marketing department of any changes to their contact information by calling 1-800-267-6847 or in writing at: Edvantage, c/o Marketing Dept., P.O. Box 218, 125 Northfield Drive West, Waterloo, ON N2J 3Z9 if they still wish to be entered into the Reward Program.
This New Membership Reward Program is subject to all applicable federal, provincial and municipal laws and regulations, is void where prohibited by law and is governed by Canadian laws and the laws of the provinces of Ontario and Alberta. If any provision of these rules is found by any court of competent jurisdiction to be unenforceable, all other provisions will remain in full force and effect.
This Reward Program is conducted in English (which will be given its everyday ordinary meaning), and any and all disputes will be resolved in the appropriate courts of the Province of Ontario, Canada exclusively, and subject to the laws of Ontario without regard to its conflict of laws principles.

Caution: Any attempt by an Edvantage Member or their spouse and/or dependent to deliberately damage any website or undermine the legitimate operation of the New Membership Reward Program is a violation of criminal and civil laws. Should such an attempt be made, OTIP and/or TW Insurance reserve the right to seek damages from any such member to the fullest extent permitted by law.
